Title I, Part D Consolidated State Performance Report Data Reporting Form
2025 CSPR Data Collection Tool (2023-24 data)
Background:
The 鶹 (鶹) is required to annually collect data for the Consolidated State Performance Report (CSPR) submission to the United States Department of Education.The CSPR Data Collection Tool must be completed for each neglected or delinquent facility under your corporate umbrella that receives Title I, Part D funds in New York State. Agencies/institutions with multiple sites must submitone collection form foreachٱ.The reporting period to be used on the collection form must beJuly 1, 2023, through June 30, 2024.
Instructions:
The 2025 CSPR data is being collected through the 2025 CSPR Data Collection Tool.When you open the file, you may see a security warning.Click on the “Enable Content” box in the yellow warning line.
NOTE:The 2025 CSPR Data Collection Tool includes data validation checks. Data validation checks prevent the facility from entering an invalid value and will give an error message.
The variables with data validation checks are:
Cell B18: The Facility cannot enter a value greater than 365.
Cell B19: The facility cannot enter a value greater than the average length of stay in cell B18.
Please ensure that the workbook is filled out completely. Each indicator must be addressed. If the indicator does not apply, put a zero in the workbook for that line. Where necessary, write an explanation in the comments section.
Once the form is complete save the file with the following naming convention:CSPR2025-[INSERT FACILITY NAME]. For example, CSPR2025-Safe Space Children’s Services. The form can be emailed toND@nysed.gov (link sends e-mail). The completed file is due to 鶹 by close of business,on Thursday, October 3, 2024.
